Adoption Form

Thank you for considering adoption through Rescue Lyfe! Every adoption gives a deserving dog a second chance at love and a place to call home—and makes room for us to rescue another in need.

The $250 adoption fee is required in full when you sign your Adoption Contract. Payment must be received before your dog can go home with you. Signing the contract does not replace or defer this payment requirement.

Our adoption fee helps cover a portion of the veterinary care, vaccinations, and transition costs involved in preparing each dog for their forever home. We keep this fee as low as possible to help more dogs find loving families.

If you feel moved to give beyond the required $250 fee, additional donations are always welcome and help us care for more dogs in need.
